Abstract: 

"The Hospitalized Frail Elder" is the leading presentation in CHAMP (Curriculum for the Hospitalized Aging Medical Patient). This presentation and teaching materials touch upon the four course themes in CHAMP: identifying vulnerable elder patients, preventing hazards of hospitalization, improving palliative and end of life care, and managing transition of care. Teaching materials include: slide presentation with speaker's notes, bibliography, bedside teaching triggers highlighting salient clinical questions and teaching points, sample session evaluation form, and a user's guide and overview of CHAMP.

CHAMP is a multi-tiered educational project with a faculty development program at its core. Hospitalists, general internists, and family practitioners who teach residents and medical students are the targeted audience. CHAMP consists of 15 geriatric topics and a mini-course entitled, "Teaching on Today's Wards," focusing on advanced clinical teaching skills and teaching across the ACGME Core Competencies.

Educational objectives: 

The overall educational goal of CHAMP is to improve the quality and quantity of geriatric practice and teaching in the inpatient setting. The specific learning objectives of "The HOSPITALIZED OLDER ADULT - IDENTIFICATION AND ASSESSMENT" are listed below.

CHAMP materials on "The Hospitalized Frail Elder" train faculty learners to:

  1. Recognize the profile of the frail or vulnerable aging hospitalized patient.
  2. Identify the impact of hospitalization on the frail older adult with regard to cognitive and functional decline, hazards of being hospitalized and mortality and the need to screen for cognitive and functional decline.
  3. Screen for cognitive decline using the Folstein MMSE or the Mini-Cog.
  4. Screen for functional decline using the Get-up-and-go test and screen for ADL and IADL impairments.
  5. Identify preventative measures that can be implemented in the frail aging hospitalized population.
Additional information/Special implementation requirements or guidelines: 

The CHAMP Faculty Development Program has trained 12 geriatricians, 19 general internists, 2 hospitalists, and 5 fellows at the University of Chicago.
